Scholarship Program
Dr. Heather Brown is happy to provide $500 scholarships to four qualifying high school seniors interested in pursuing a career in dentistry. Awards will be based on academic achievement, extracurricular activities, and community involvement.
Please download and print out the application form and questionnaire. Please submit your completed application form & questionnaire, along with a sealed transcript, a letter of recommendation, and letter of acceptance to the college you will be attending.
Scholarship application packets are due in the office by 5:00PM on May 6, 2013. Incomplete or late applications will not be reviewed.
*Any high school senior is eligible for the scholarship. Award is not limited to former or current patients of Dr. Heather Brown.
